WHAT IS IT?


An individual student nomination form which provides context of a students needs to HAD in Schools. Captures student information, reason for nomination, current support in place, and consent status.

One form is to be completed per student participating in a HAD in Schools Program.

HAD in Schools recommends submitting forms for ‘back up’ students who may participate in a session if one of the originally nominated students is absent.

when TO USe IT?


Once you have identified students who you believe will benefit from the HAD in Schools Program, please nominate them via the below form.

Ensure that you have also requested parent/guardian consent either prior to or very soon after completing this form.

Why collect this information?


Who completes it?


Nomination forms should be completed by one of:

  • Teacher of the student

  • Wellbeing Manager

  • A teacher/staff member who knows the student well

  • OHSC Team Member or Manager

what happens next?


Our team reviews student profiles to confirm student suitability and mentors prepare tailored session plans before program delivery begins.

In the unlikely event that a student is deemed unsuitable for our program, we will contact you to discuss alternative students to be nominated.

Capturing relevant student information prior to participation in the HAD in Schools Program is essential to ensure the safety, wellbeing, and suitability of each student, and to support effective program delivery within a school environment.

  3. To ensure the program is a safe and appropriate fit for each student

  4. To support child safeguarding and clear supervision boundaries

  5. To help mentors engage students appropriately from day one

  6. To enable effective incident response and escalation if required

  7. To maintain clear communication and accountability with schools

  8. To meet school governance, insurance, and compliance requirements

  9. To support program evaluation, allowing us to compare pre- and post-program data and continuously improve our model while demonstrating program outcomes

All student information is handled confidentially, stored securely, and used only for program delivery, safeguarding, evaluation, and duty of care, in line with our Privacy Policy.
