Parent/guardian consent form

Your child’s school is proud to offer them access to the HAD in Schools Program!

The program focuses' on helping students build confidence, social skills, and emotional wellbeing (amongst other things) through relaxed, low-intensity physical activity and meaningful conversation. The aim is simple — to strengthen student engagement and to help each learner meet (or even exceed) both school and personal expectations.

Mentors are carefully selected by HAD in Schools not only for their sporting experience — each being an elite or aspiring-elite athlete in their own right — but more importantly, for their character, empathy, and ability to connect with young people. While their sporting background helps them build instant rapport, it’s their genuine, down-to-earth nature and positive influence that make the biggest impact. They’re relatable role models who bring out the best in the students they work with, and they all hold a current Working With Children Check.

The sessions take place during school hours and on school grounds, and we’ll ensure they’re run in a safe & supervised environment.

If you’re happy for your child to take part, please complete the consent form below:

  • “At the start of 2025, I had a distant, sad and withdrawn student. Now I have a happy, engaged, and confident student who can acknowledge and reflect on his actions and is willing to talk through conflicts”.

    —Teacher, Birmingham Primary School

  • "We’ve seen the positive impact on our students, and the program is now strongly recognised and endorsed by both staff, students and parents"

—Youth Counsellor, Upper Yarra Secondary College

  • "This program has completely transformed my child. Previous to working with Georgia, she would get stressed, anxious and was struggling with the tools to help. Through Georgia’s lived experience in the sporting world, our daughter instantly felt a connection with someone who has felt this way at times. Now she has better coping mechanisms that she puts to great use"

    -Parent, Birmingham Primary School
