Student nomination form


WHAT IS IT?


The form your school uses to refer individual students to HAD in Schools mentoring. Captures student information, reason for referral, current support in place, and consent status.

One form is to be completed per student participating in a HAD in Schools Program.

HAD in Schools recommends submitting forms for ‘back up’ students who may participate in a session if one of the originally nominated students is absent.

when TO USe IT?


After identifying suitable students (via SAEBRS, teacher observations, or existing wellbeing data) and obtaining parent/guardian consent.

Who completes it?


Nomination forms should be completed by one of:

  • Teacher of the student

  • Wellbeing Manager

  • A teacher/staff member who knows the student well

what happens next?


Our team reviews student profiles to confirm they align with our Tier 2 suitability and mentors prepare tailored session plans before program delivery begins.

In the unlikely event that a student is deemed unsuitable for our program, we will contact you to discuss alternative students to be nominated.

Parent/Guardian Consent form

WHAT IS IT?

The official consent form parents/guardians complete to authorise their child's participation in HAD in Schools mentoring.

Obtaining consent via this form is a compulsory part of our program.

how to use it?

Parents/guardians complete the consent form online through our website, where responses are collected and tracked centrally by HAD in Schools. We confirm all required consents have been obtained before program delivery begins.

Need a Physical consent form instead? Email info@hadinschools.com.au and we'll provide a printable version for your school's records.

Parent/Guardian information Letters

WHAT IS IT?

An editable letter template explaining to parents/guardians that their child has been nominated for the HAD in Schools Program and directing them to learn more through our website and complete the consent form.

how to use it?

Send this letter to parents/guardians of students you've identified as suitable for the program. The letter can be customised with your school logo.

WHAT IS IT?


An editable template for the rare situation where a student participates in a session to fill in for an absent nominated student. The letter provides context about the program, explains why their child was included, and requests consent for any future participation.

how to use it?


Send this letter to parents/guardians whose child unexpectedly participated in a session due to another student's absence.

Note: This is uncommon - we recommend schools nominate 1-2 additional students as backups to minimise the need for substitute participants.




SAEBRS SCREENING TOOL

WHAT IS IT?


The Social, Academic, and Emotional Behaviour Risk Screener (SAEBRS) is a Department of Education research-validated tool that helps schools identify students with behavioural and emotional support needs.

HOW TO USe IT?


Teachers rate students across social, academic, and emotional behaviours. Results indicate "at risk" or "not at risk" ratings to inform wellbeing discussions. Takes 1-3 minutes per student.

who is it for?


Optional resource for schools that haven't yet categorised students or want evidence-based screening support to identify suitable participants for Tier 2 mentoring programs.

WHAT IS IT?


Single-page display flyer designed to be printed and displayed throughout school facilities - noticeboards, staff rooms, corridors, reception areas, and classroom doors. The flyer provides a brief, accessible overview of HAD in Schools.

Why use it?


This flyer is a quick and informal way to inform the school community that your school will be hosting the HAD in Schools Program! It will also ensure that there is no uneasiness if students/teachers/parents see our Mentors in uniform throughout the school.

Why use it?


This info Sheet can be used as a concise briefing document for Teachers/Parents/Leadership who are unaware of the program but want to briefly understand the core concepts.


our commitment to student safety

HAD in Schools operates with comprehensive safety protocols and privacy policies and strict compliance with child safety standards. Schools can trust that we meet all regulatory requirements and prioritise student wellbeing above all else.


working with children checks

All HAD in Schools mentors hold current Victorian Working with Children Checks (WWCC) and undergo a National Police Check.

  • Every mentor is screened through the Victorian Government's WWCC system (or state equivalent)

  • Checks are renewed and verified regularly to ensure ongoing compliance

  • WWCC card numbers and expiry dates available for school verification upon request

  • Mentors understand and comply with Victoria's Child Safe Standards

School Verification:
We provide WWCC documentation as part of our onboarding process with new schools. Contact us to request copies for your records.


Child Safety & Mandatory Reporting

If concerns arise:
Mentors immediately alert your school wellbeing contact, who activates the school's child protection and crisis response protocols as appropriate. Schools, as the employing authority with mandatory reporters on staff, manage formal reporting obligations to Child Protection or police.

Our mentors comply with:

  • Victoria's Child Safe Standards - Understanding and implementing the 11 Child Safe Standards required for organisations working with children

  • Failure to Protect & Disclose obligations - All Victorian adults must report to police if they reasonably believe a sexual offense has been committed by an adult against a child under 16

  • Duty of Care reporting - While not mandatory reporters under the Children, Youth and Families Act 2005, mentors exercise their professional duty of care by reporting suspected child abuse or neglect to appropriate authorities (Child Protection, police, or school wellbeing teams)

  • Trauma-Informed Practice - Recognising signs of trauma and abuse, responding with sensitivity, and following appropriate referral pathways

  • Safeguarding Protocols - Maintaining professional boundaries, documenting concerns, and following school-specific child safety policies


Risk Management & Incident Protocols

Session Supervision:


Mentors work within school supervision policies (e.g., door-open protocols, visible spaces, designated areas). Sessions are conducted in line with your school's existing safety procedures.

Emergency Procedures:


Mentors are all first aid trained, and have access to school first aid, emergency contacts, and evacuation plans. All incidents are documented and reported to school leadership immediately.

Privacy & Confidentiality:


Student information is handled in accordance with privacy legislation. Session observations are shared with designated school wellbeing contacts only. Personal student information is stored securely and never shared externally without consent.

Incident Reporting:


Any safety concerns, injuries, behavioural incidents, or student disclosures are reported to the school wellbeing contact immediately. Schools retain full case management and duty of care responsibilities.


Scope of Practice & Boundaries

What our mentors DO:


  • Provide relationship-based support, encouragement, and positive role modeling

  • Create safe spaces for students to build confidence and social skills

  • Use movement and conversation to support emotional awareness and regulation

  • Alert school staff immediately if concerns arise

  • Immediately report child safety concerns

What our mentors DO not do:


  • Diagnose mental health conditions

  • Provide therapy, clinical counseling, or medical advice

  • Manage crisis situations independently

  • Replace school psychologists, counselors, or external clinical services

  • Make decisions about student welfare or case management

if student needs exceed mentoring scope, had in schools will work with your wellbeing team to off-board the student from our program so they can undertake appropriate clinical or therapeutic referrals.
